There are glitches in the graphics that look like a black texture overlay.

Correct Rendering of the Scene
What steps will reproduce the problem?
Run Tomb Raider Anniversary with default settings
Dolphin Version:
3.5-316 x64
3.5-202 x64
Did the game ever work correctly?
The problem can be solved by completely disabling Graphics->Hacks->EFB-Copies.
Note: To Ram will not fix the issue
Operating System and version:
Ubuntu Linux 12.10 x64
Any other relevant information:
HD4850 with fglrx
Intel HD4000 with mesa/i915

Can you upload a 2-frame fifo log showing the issue, including a reference screenshot? Please enable EFB to RAM (uncheck the "Cache") and set Texture Cache accuracy to safest for that.

I can only reproduce it with Opengl and fractional efb scale(auto). Any other setting and/or graphics backend works fine. Use any other value of efb scale than auto fractional temporally. Perhaps Neobrain can figure this out...

The issue seems to be valid as it can be reproduced in Video_Software. Also, enabling EFB copies to RAM makes the issue appear in the hw renderers, too.

Fwiw, I wasn't able to test this on D3D backends.
